Ohio Scales

The Ohio Scales Outcomes instruments are designed to measure the changes to your, or your child's, life as a result of mental health treatment. The scales can be used to measure and monitor certain aspects of your life on your own, or as part of treatment with in Ohio's public mental health system.

Notice to out-of-Ohio provider agencies: You must pay a low, one-time fee for the use of the Ohio Scales. Payment should be made to the Southern Consortium for Children; please contact Ben Ogles at ogles@ohio.edu for more information. Use by youth and family members is free of charge.


Please Note: Use of the Ohio Outcomes instrument on Network of Care does not convey the results of the survey to your provider agency in Ohio. Please feel free to print out a copy of the response to bring to your provider in person.
